LLC is committed to protecting the privacy and security of your protected health information (PHI).

Your child’s health information may be used to evaluate symptoms, provide medical advice, and

coordinate care with other healthcare providers when appropriate, including during in-home or

telemedicine visits.

Health information may be used to process payment for services provided by MamaMedics LLC.

Healthcare Operations

Health information may be used for internal operations such as quality improvement, training, and

administrative management of the practice.

• Public health reporting to prevent or control disease, injury, or disability.

• Reporting suspected child abuse or neglect when required by law.

Telemedicine

MamaMedics LLC may provide care through telemedicine services. During these visits, protected

health information may be transmitted electronically through secure systems in order to evaluate and

treat your child.

Your Rights• Request access to or copies of your child’s medical records.

• Request corrections to health information you believe is inaccurate.

• Request confidential communication methods such as email.

• Request restrictions on certain uses or disclosures.

• Receive a copy of this Notice of Privacy Practices.

Uses and Disclosures Requiring Authorization

MamaMedics LLC will obtain written authorization before using or sharing health information for

purposes not described in this notice. Authorization may be revoked in writing at any time.

MamaMedics LLC reserves the right to update this notice at any time. Updated versions will apply to all

health information we maintain and will be available on our website.
